Reviewme Pty Ltd collects personal and health information that is necessary to provide professional Home Medication Review services. This may include patient identification details, medication histories, referring GP and pharmacy details, and consultation notes and recommendations. Information is obtained directly from patients, referring general practitioners, and pharmacies through written consent prior to conducting the review.

Use and Disclosure of Information

Information collected by Reviewme Pty Ltd is used solely for the purpose of conducting Home Medication Reviews and communicating relevant recommendations to the patient’s GP and referring pharmacy. Patient information is disclosed only to verified healthcare providers involved in the patient’s care. Reviewme Pty Ltd does not use or share any patient data for marketing, promotional, or unrelated purposes.

Confidentiality and Staff Obligations

Only authorised pharmacists and support staff employed or contracted by Reviewme Pty Ltd have access to patient information. All personnel are bound by confidentiality agreements and internal privacy procedures to ensure compliance with legal and professional obligations, including those of the Pharmacy Board of Australia and AHPRA.

Data Security

Reviewme Pty Ltd maintains secure on-site storage for all records and information. Security measures include password-protected systems, physical access controls within the office, and secure backups on encrypted local drives. Patient information transmitted via email or fax is sent only to verified healthcare providers using secure and confidential channels to minimise the risk of unauthorised access.

Data Retention and Disposal

Reviewme Pty Ltd retains records and reports for as long as required under Australian healthcare and pharmacy regulations. When records are no longer legally required, they are securely destroyed in accordance with professional and statutory guidelines.

Compliance

This policy has been developed in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and the professional confidentiality standards set by the Pharmacy Board of Australia and AHPRA.
