For SRS and NDIS homes
Residents of supported residential services (SRS) and NDIS supported accommodation can be eligible for a Home Medicines Review. We work with each resident's GP and bring an accredited pharmacist to you, grouping visits into one day rather than a string of interruptions.
Residents stay put. The pharmacist comes to them.
Several residents seen in one scheduled visit.
We work with each resident's own doctor.
All we need from you is a list of interested residents and a day that suits. Running several NDIS houses? Send us the list of houses and we'll take it from there.
1
We learn about your home or houses, your residents and how medicines are managed today.
2
With each resident's consent, we contact their GP. The GP decides whether to refer and issues the referral.
3
An accredited pharmacist reviews each referred resident, taking about 45 to 60 minutes per person.
4
Each GP receives the pharmacist's recommendations. With consent, we can share relevant outcomes with your staff.
Residents often see more than one prescriber. A review can pick up duplications and interactions.
Clear advice from a pharmacist helps the people who assist with medicines every day.
A clear, professional step in caring for someone they love.
Tell us about your home or houses and we'll call to plan a first visit day. Or call 1300 275 467, Monday – Friday, 9am – 5pm.
