GP referral
The patient's GP decides a review is needed and issues the referral.

Our office calls the patient and picks a time that suits them.

Home visit
An accredited pharmacist reviews every medicine — usually 45 to 60 minutes.

Report to GP
The GP receives the findings and recommendations to discuss with the patient.

Your doctor has referred you for a Home Medicines Review. Here's what to expect.
Can I choose when the pharmacist visits?
Yes. Our office will call you to arrange a date and time that suits you.
What do I need for the visit?
Just the medicines you take — including vitamins and over-the-counter products. The pharmacist will ask a few questions and write a report for your doctor.
How long does it take?
Usually 45 to 60 minutes, depending on how many medicines you take.
Who is eligible?
People living at home who are at risk of problems with their medicines — for example, taking five or more regular medicines, recently out of hospital, or with recent changes to their medicines. Your GP decides whether a review is right for you.
Is there a cost?
No. Home Medicines Reviews are government-funded, so there is no charge to eligible patients.
Can the pharmacist speak my language?
Our pharmacists speak a range of languages. Let our office know your preference when we call.
